To apply for a visa, the following documents must be submitted:
Passport that must be valid for at least three months beyond the date of the holder's departure from Ukraine, containing at least two empty pages and issued within the previous 10 years.
Application form duly completed and signed by the applicant in person. Visa application forms of minors (below the age of 18) need to be signed by a least one of their parents or the person exercising legal guardianship.
One recently taken passport photo (35x45 mm)
Health insurance valid for the entire stay and accepted in Ukraine. The insurance must cover the minimum amount of EUR 30’000.00.
If the visa is requested with a longer validity and multiple entries, the travel insurance only needs to cover the first trip. The applicant acknowledges that the necessary insurance protection must also be available for each additional trip.
Solvency proof (bank account statement for the last two months, bank letter confirming the balance of the account). Each person must be able to prove approx. CHF 60.00 (or equivalent in another currency) per day of stay + CHF 300.00.
The email confirmation of an agreed appointment.
The visa fee must be paid after the interview with the responsible consular employee of the embassy.
For citizens of third countries a residence permit for the Swiss Confederation (+ copy) is required, otherwise the visa must be applied for in their home country.
(B-01) - Documents in relation to the onward journey to the final destination (visa or entry permit for the third country / destination country, tickets for the onward journey);
(B-02) - Proof of the transit transportation of goods and passengers by road vehicles;
(B-03) - International transportation license issued by the competent authority of a foreign country;
(C-01) - Original of the Invitation letter from a company in Ukraine (on the official company letterhead), signed by an authorized person, with the following information: state registration number of the company; reference number, date and signature; surname, first name, gender, date of birth and nationality of an invited person, passport details, place of residence; purpose of stay, length of stay and number of entries; guarantee of covering the costs of the stay and departure of the invited person from Ukraine;
(C-02) - Original of the notarized invitationfrom a citizen of Ukraine or a foreigner / stateless person who has a residence permit in Ukraine, with the following information: name, surname, passport details, address and telephone number of the inviting person (host); name, surname, date of birth, place of birth, gender, nationality, passport number, address of the invited person (guest); place of residence in Ukraine, duration of stay and number of desired entries; guarantee of covering the costs of the stay and departure of the invited person from Ukraine; copy of the Ukrainian passport of the inviting person (if the inviting person is a national of Ukraine) or copies of the passport of the foreigner / stateless person and his Ukrainian residence permit (if the inviting person is a foreigner / stateless person);
(C-03) - Invitation letter from a government agency, institution, company or organization;
(C-04) - Contract for the transportation of goods and passengers and the license for international transport;
(C-05) - Certificate of a person of Ukrainian descent. Applicant’s family provides documents on their status and family relation to the applicant;
(C-06) - Proof that the applicant’s main purpose of journey to Ukraine is tourism (confirmed hotel booking, round-trip ticket, stay program, information about the travel agency, information on places of interest, etc.);
(C-07) - Original of the invitation letter from Ukraine’s hospital/medical treatment institution;
(C-08) - Proof of foreign investment in accordance with the legislation of Ukraine;
(C-09) - Original of the invitation letter from a cultural organization, scientific institution, educational institution, sports organization;
(C-10) - Original of the invitation letter from a religious organization agreed with the state authority;
(C-11) - Original of the request from the foreign media according to which a staff member travels in his/her professional capacity;
(C-12) - Original of the invitation letter from foreign states’ authorities / international organizations.
(D-01) - Copy of the immigration permit issued by the State Migration Service of Ukraine;
(D-02) - Proof that the applicant is a family member of a refugee recognized in Ukraine *;
(D-03) - Verbal note from a relevant diplomatic mission / international organization;
(D-04) - Certified copy of the work permit for foreigners / stateless persons;
(D-05) - Invitation letter from a state agency, state enterprise or state organization in Ukraine which is a recipient of an international technical assistance project;
(D-06) - Original of the invitation letter from a religious organization duly registered in Ukraine accompanied by a letter issued by Ukraine’s state authority which has registered that religious organization;
(D-07) - Original of the invitation letter from a foreign public non-governmental organization duly registered in Ukraine;
(D-08) - Original of the invitation letter issued by a foreign company duly registered in Ukraine;
(D-09) - Original of the invitation letter issued by a branch of a foreign bank registered in Ukraine;
(D-10) - Original of the invitation letter from Ukraine’s relevant government authoritywhich implements respective cultural, educational, scientific, sport or volunteering programs or an invitation letter from an entity/institution/organization engaging volunteers in their activities accompanied by the copy of that entity/institution/organization’s state registration certificate;
(D-11) - Original of the request from the foreign media according to which a staff member travels in his/her professional capacity for a long stay;
(D-13) - Original of a properly issued invitation to study;
(D-14) - Documentary proof of the existence of marriage to a citizen of Ukraine *;
(D-15) - Documentary proof of the existence of marriage to a foreigner or a stateless person who has temporary residence permit in Ukraine. Copy of the residence permit, as well as confirmation of the necessary financial support for the applicant *;
* Foreign documents and official certificates must always be provided with an apostille. The apostille confirms the authenticity of the document / official certificate. If you do not know which authority confirms the authenticity of the certificate in the form of the apostille, you can contact the authority that issued the document. The apostille must be attached to the original of the certificate.
All documents, certificates and other documents submitted as part of a long-term visa application must be translated into Ukrainian by an approved translator. An apostille must be translated as well.
